On or about 11/19/2017, an employee working at Shop 'N Save Warehouse Foods Inc., located at 100 Jungerman Road, St. Peters, MO, was exposed to eye and face hazards. Specifically, the employee serviced lead acid batteries in a Crown Walk-Behind Powered Industrial Truck, Model SHR5520-30. The employee did not wear a face shield or safety glasses; therefore, the employee's eyes and face were exposed to chemicals hazards.

On or about 1/18/2017, employees at Shop 'N Save Warehouse Foods Inc., located at 100 Jungerman Road, St. Peters, MO, were operating an unsafe powered industrial truck. A Crown Walk-Behind Powered Industrial Truck, Model SHR5520-30, was unsafe and not removed from service until restored to safe operating condition. The unit had the following issues: a) The operator's protective cage was missing. b) While the operators moved the unit forwards or backwards, the unit would unexpectedly, abruptly stop causing the loads to shift.
