On or about January 25, 2017, an employee, not wearing a hard hat, was working on the ground below other employees on the roof, the employee was exposed to head injury from falling tools and or debris from the employees above.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system, or personal fall arrest system, nor are employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on January 25, 2017, when employees are working on a residential roof at 11 3rd Street, Saint Augustine, Florida 32080 without fall protection and were exposed to falls of approximately 18 feet. b. The employer fails to assure effective protection against falls as required by the standard, as demonstrated on January 25, 2017, when employees are working on a residential roof at 11 3rd Street, Saint Augustine, Florida 32080 without fall protection and were exposed to falls of approximately 8 feet. J. Adams Roofing, Inc., previously committed a repeat violation of a similar standard on November 3, 2016, with respect to a workplace at 3308 Coastal Highway, Saint Augustine, FL 32084, which became a final order on December 27, 2016, (Inspection No. 1188618). J. Adams Roofing, Inc., previously committed a serious violation of a similar standard on August 11, 2016, with respect to a workplace at 105 1st Street, Saint Augustine, FL 32084, which became a final order on October 12, 2016, (Inspection No. 1168786).
