29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: ESTABLISHMENT: The employer did not ensure through visual observation, specific instructions and other effective means that the employees performing cleaning of kettles and machinery with hot water were wearing appropriate protective equipment, such as (but not limited to) protective pants at all times. On or about 01/13/2017, one of employees was splashed with hot water and sustained severe burns to right arm and right foot while the employee was trying to move a trough containing hot water without wearing appropriate protective pants.
