29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: Workplace: The guard on the main chain drive on the Coleburn pie making machine was removed while the machine was in operation, exposing employees to amputation hazards. On or about August 4, 2020, the left index finger of a mechanic working very close proximity from t he above machine was pulled into the unguarded sprocket and chain, resulting in a finger amputation of the second digit at the level of the distal phalangeal tuft requiring hospitalization.
