29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s).      On or about January 25, 2107:    a)  In the production area  where employees were exposed to crushed-by hazards when operating a Bliss single hinge punch press #A without a guard.    b) In the production area where employees were exposed to crushed-by hazards when operating Bliss strike plate press without an adequate guard.    c)  In the production area where employees were exposed to crushed-by hazards when operating the Danly notch press, Unit 13 without an adequate guard.      d) In the production area where employees were exposed to struck-by hazards when operating the Danly notch press, Unit 14 without an adequate guard.    Frameworks Manufacturing Inc  was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1910.212 (a)(3)(ii), which was contained in OSHA inspection number 905778, citation number 1, item number 3 and was affirmed as a final order on 6/11/2013, with respect to a workplace located at 3801 Yale Street, Houston, Texas.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.    This violation occurred on or about January 19, 2017, at the facility where an employee suffered an amputation injury as a result of a work-related injury that was not reported within 24 hours.
