29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s).     On or about March 8, 2017:    a) In the production area where an employee was exposed to a crushed-by hazard when operating the FYCO tool and die hinge jam punch press without it being guarded.     b) In the production area where an employee was exposed to a crushed-by hazard when operating the header jam punch press that had an inadequate guard.    Frameworks Manufacturing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA Inspection number 905778, citation number 1, item number 3 and was affirmed as a final order on 6/11/2013, with respect to a workplace located at3801 Yale Street, Houston, Texas.
