29 CFR 1910.1200(h)(1):     Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(2) and (h)(3) on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    (a) Employees who were exposed to hazardous chemicals (e.g., welding fumes, gasoline, diesel fuel, solvents, motor oil, etc.) while performing maintenance activities had not been provided information and training concerning the following:    INFORMATION    * Requirements of the hazard communication standard    * Operations in their work area(s) where hazardous chemicals are present    * Location and availability of the written hazard communication program, including the required list of hazardous chemicals and material safety data sheets/safety data sheets    TRAINING    * Physical and health hazards of hazardous chemicals    * Measures employees can take to protect themselves from chemical hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment    * The details of the hazard communication program developed by the employer, including an explanation of the labeling system, the material safety data sheet/safety data sheet, and how employees can obtain and use this hazard information    * As of 1/18/17, employees had not been trained on the new chemical container label elements and the new format and types of information on each section of safety data sheets (SDSs) with respect to revisions to the standard in accordance with the Globally Harmonized System of Classification and Labeling of Chemicals (GHS).      ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:  (a) On or about January 18, 2017, maintenance employees were potentially exposed to workplace hazards (including but not limited to eye, skin, and respiratory irritants) for which the employer did not have a written workplace hazard assessment.    Notice #001, Item #1, 29 CFR 1910.132(d)(1) as required  by 29 CFR 1960.8(b): The employer had not conducted a workplace hazard assessment and verified completion of it through a written certification. The employer was cited for a violation of the same standard on 11/12/14, Inspection #977444, 12/05/14 of final order/settlement, and was abated by completing a hazard assessment.
