29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems; safety net systems; or personal fall arrest systems.   a) South end of the Maintenance Office Building grounds: When employees are entering/exiting the door to the office building and/or using the concrete walkway to the parking lot they are exposed to falling 14 feet 9 inches to the ground below.   ABATEMENT DOCUMENTATION IS REQUIRED.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  a) Lifesaving Station Worksite: A Komatsu Forklift USNPS 59616 used at the site to load and unload supplies was equipped with a nonfunctioning horn.   ABATEMENT CERTIFICATION IS REQUIRED

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  a) West wall of the Maintenance Office Break Room: An energized 120 volt relocatable power tap was found to be screwed into a wall requiring the use of hand tools to remove it. This is a violation of it's listing due to it no longer being relocatable.  b) North wall of the Lifesaving Station Maintenance Shop: A Titan tankless water heater designed and listed to have its electrical power cord to be plugged directly into a wall mounted receptacle outlet was instead plugged into a multi tap.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service:    a)    Southwest corner of the Lifesaving Station Maintenance Shop: The electrical cord to the Makita 12 inch metal cutting saw, model number LC1230; was found to have a 1/2 section of the outer insulation exposed near the plug-end and a couple inches to the plug-end was another cut which went around the entire circumference of the cord exposing the wires underneath to contact with the elements and employees.

29 CFR 1960.59(a): The Agency did not provide appropriate safety and health training for employees:   a) Southwest corner of the Lifesaving Station Maintenance Shop:  The employer did not ensure employees were trained to recognize that an unstable, wobbly  three legged worktable used to support a metal cutting saw during cutting operations was a safety hazard and can not be used.   ABATEMENT CERTIFICATION IS REQUIRED
