29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    Warehouse, refinish area and paint booth: on or about January 26, 2017, employees were spraying flammable and hazardous chemicals such as, but not limited to, Mohawk paints, lacquers, toners from aerosol cans and from pressurized spray-gun in the paint booth to refinish furniture surfaces. Employees were required to wear provided half-face tight-fitting 3M respirators without medical evaluations and fit testing performed prior to use. Written Respiratory Protection Program was not developed and documented for workplace-specific procedures and training pertaining to use, cleaning, and maintenance of equipment to ensure safety.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.213(c)(1): Circular handfed ripsaw(s) were not guarded by an automatically adjusting hood which completely enclosed that portion of the saw above the table and above the material being cut:  South, warehouse, wood shop repair area, Rigid table saw: on or about January 26, 2017, employees were hand-feeding wood stock for ripping on the equipment with a missing point-of-operation blade guard. Employees had their fingers and hands exposed to contact with rotating blade.  There is no abatement certification or documentation required for this item.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    South, warehouse, wood shop repair area, Rigid table saw - on or about January 26, 2017, employees were coming in contact with the saw's metal frame for hand-fed wood stock ripping and switching the equipment "on" and "off". Employees were exposed to an electric shock/electrocution hazard due to the use of an ungrounded 120V AC cord where the grounding prong was removed at the plug.
