29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition: On or about January 27, 2017, employees were exposed to trip hazards while traveling through the grocery store back room in the following areas: a) The square drain plate cover located approximately in the south part of the grocery back room was not securely fasten to the floor. b) Located approximately in the central area of the grocery back room, the concrete floor surrounding the drain was sunken in. There was no square drain plate cover drain over this drain. The employer did not ensure that floor was maintained in a safe condition. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c) .

29 CFR 1910.157(c)(1): The employer shall provide portable fire extinguishers and shall mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury. On or about January 27, 2012 a blocked fire extinguisher was observed by the staircase in the grocery backroom. It was not readily accessible in the event of a fire. Abatement certification is required in accordance with the requirements of 29 CFR 1903.19(c)

29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling. On or about January 27, 2017 at the computer workstation located in the employee break room, employees were exposed to a shock hazard due to an extension cord taped to the wall and wrapped around the doorway. The employer failed to protect employees from shock hazards by not ensuring the electrical equipment was not run through doorways. Abatement certification is required of this item in accordance with the requirements of 29CFR 1903.19(c).

29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used in the following manor: (A) As a substitute for fixed wiring of a structure (B) Where run through holes in walls, ceiling, or floors (C) Where run through doorways, windows or similar openings (D) Where attached to building surfaces (E) Where concealed behinds walls, ceilings, floors (F) Where installed in raceways, except as otherwise permitted in this subpart. On or about January 27, 2017 at the computer workstation located in the employee break room, employees were exposed to a shock hazard due to an extension cord being connected to a relocatable power tap (RPT), which was plug into a wall receptacle. The extension cord was also taped to the wall and wrapped around the doorway. The employer failed to protect employees from shock hazards by not ensuring the electrical equipment was used in the proper manor. Abatement certification is required of this item in accordance with the requirements of 29CFR 1903.19(c).

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: On or about January 27, 2017 sufficient access and working space was not provided or maintained for electrical panels located in the electrical and compressor rooms and equipment storage room. The space between the electrical panel and the material was less than three feet. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19 (c).
