The caustic tank normally contains a 50% solution of sodium hydroxide that overflows to the outside of the tank into a sparger unit. The 3M dust mask selected and provided by the employer for voluntary use was not appropriate to protect employees from the hazards of exposure to sodium hydroxide resulting in employees developing chemical inhalation injuries requiring treatment, on or about 2/4/2017 and 2/5/2017. 29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeds one fourth inch:    Recent examples of the employer's violative conduct occurred on these occasions:  a) 9000 River Rd, Delair, NJ, 08110- Building 11, Maintenance Area: The tongue guard on a Baldor bench grinder model 1215W serial number F302 had an opening that measured 7/8 inches from the grinding wheel, on or about 1/24/2017.  b) 9000 River Rd, Delair, NJ, 08110- Die Correction: A Baldor bench grinder model 712 RE serial number H584 did not have a tongue guard and the opening measured 1 1/8 inches from the grinding wheel, on or about 1/24/2017.   To abate this violation, the employer must establish a system to ensure that all grinding wheels have installed tongue guards that are properly adjusted to one quarter inch from the grinding wheel during employee use.
