29 CFR 1910.134(d)(1)(i): Selection of appropriate respirators was not based on the respiratory hazard(s) to which the worker was exposed and user factors that affected respirator performance and reliability:  a) 9000 River Rd, Delair, NJ, 08110- Anodizing:  Employees were instructed to enter the caustic tank to clean out residue manually using water and shovels. The caustic tank was drained and normally contains a 50% solution of sodium hydroxide. The 3M dust mask selected and provided by the employer for voluntary use was not appropriate to protect employees from the hazards of exposure to sodium hydroxide resulting in an employee developing a chemical inhalation injury requiring treatment, on or about 2/3/2017.  b) 9000 River Rd, Delair, NJ, 08110- Anodizing:  Employees were instructed to manually clean residue around the outside of the caustic tank using shovels. The caustic tank normally contains a 50% solution of sodium hydroxide that overflows to the outside of the tank into a sparger unit. The 3M dust mask selected and provided by the employer for voluntary use was not appropriate to protect employees from the hazards of exposure to sodium hydroxide resulting in employees developing chemical inhalation injuries requiring treatment, on or about 2/4/2017 and 2/5/2017. Brass shell, paper-lined sockets, or other metal-cased sockets may not be used unless the shell is grounded:   Recent examples of the employer's violative conduct occurred on these occasions:  a) 9000 River Rd, Delair, NJ, 08110- Building 11, Die Correction Mezzanine:  Overhead fluorescent lights were not provided with a guard to protect them from accidental contact or breakage, observed on or about 1/24/2017.   b) 9000 River Rd, Delair, NJ, 08110- Building 3, Sutton Press, Extrusion Section of Press:  A metal clamp light was not provided with a guard to protect it from accidental contact or breakage, observed on or about 1/23/2017.   c) 9000 River Rd, Delair, NJ, 08110- Building 3, Sutton Press, Hot Saw Area of Press:  A metal clamp light was not provided with a guard to protect it from accidental contact or breakage, observed on or about 1/23/2017.   d) 9000 River Rd, Delair, NJ, 08110- Building 11, Die Correction: A light attached to the Victor Milling machine, #2350 was not provided with a guard to protect it from accidental contact or breakage, observed on or about 1/24/2017.   To abate this violation, the employer must ensure that the light fixture is protected from accidental contact or breakage by providing a guard or suitable fixture.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, AND PURSUANT TO 29 CFR 1903.19, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, INCLUDING THE STEPS IT IS TAKING TO ENSURE COMPLIANCE WITH THE STANDARD TO PROTECT EMPLOYEES FROM THE EXPOSURE TO ELECTRICITY AND FLYING GLASS FRAGMENTS AND INCLUDE A PHOTO OF THE GUARDING PROVIDED.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19. 29 CFR 1910.1200(f)(6): The employer does not ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with the product identifier, signal word, hazard statement(s), pictogram(s), precautionary statement(s) and name, address, and telephone number of the chemical manufacturer, importer, or other responsible party:    Recent examples of the employer's violative conduct occurred on these occasions:  a) 9000 River Rd, Delair, NJ, 08110- Building 1 Maintenance Area work cart: The employer did not ensure that a spray bottle containing a brown liquid was labeled with the information required by the standard, on or about 1/23/2017.   b) 9000 River Rd, Delair, NJ, 08110- Building 11 Maintenance Area work table: The employer did not ensure that a spray bottle containing an unknown chemical was labeled with the information required by the standard, on or about 1/24/2017.   c) 9000 River Rd, Delair, NJ, 08110- Building 11 Maintenance Area floor under work bench: The employer did not ensure that a one gallon jug containing an unknown chemical was labeled with the information required by the standard, on or about 1/24/2017.   To abate this violation, the employer must ensure that all containers of chemicals are labeled according to the requirements of the standard and that employees are aware of the requirements and only use properly labeled containers. 29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:  9000 River Rd, Delair, NJ, 08110: As of 4/12/2017 the employer had not recorded the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2017.  a) Anodizing: On 2/3/2017 an employee was inside of the caustic tank removing sludge and received chemical burns on his left leg and ankle. The employee received medical treatment with the use of a prescription antibiotic cream on 2/7/2017.  b) Anodizing: On 2/3/2017 an employee was inside of the caustic tank removing sludge and received chemical burns on his hands and scalp. The employee received a prescription for an antibiotic on 2/7/2017.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
