29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  a). On or about 1/25/17, in the Sub-Basement, employees were handling chemicals such as Sika Anchor Fix_3001. The product is a two part epoxy that contains chemicals, such as, but not limited to, ethylenedioxydiethyl dimethacrylate and dibenzoyl peroxide. The product is a skin sensitizer. The employer did not asses the workplace to determine that PPE such as nitrile gloves and safety goggles/glasses may be required. The employer provided safety glasses and gloves on a voluntary basis.

On or about 1/25/17, in the Sub-Basement, employees were handling chemicals such as Sika Anchor Fix_3001. The product is a two part epoxy that contains chemicals, such as, but not limited to, ethylenedioxydiethyl dimethacrylate and dibenzoyl peroxide. The product is a skin sensitizer. 29 CFR 1910.1200(h)(2)(iii): The employer did not provide information to the employees as to the location and availability of the written hazard communication program, and material safety data sheets required by 29 CFR 1910.1200:   a). On or about 1/25/17, in the Sub-Basement, employees were handling chemicals such as Titebond III Premium wood glue. The product contains chemicals, such as, but not limited to, aluminum chloride. The employer maintained SDS and a hazard communication, however, the employees were unaware of the location.
