On or about 01/31/17 - Glass disposal dock, the doorway (dock) was inadequately guarded to protect employees from falling into the bin and onto glass.

On or about 01/31/17 - Cutting Table, personal protective equipment such as face shield, neck and torso cut protection were not utilized where employees were exposed to shards of glass while breaking and disposing of remnants/drops of glass in the hopper. b. On or about 01/31/17 - Dock, personal protective equipment such as face shield, neck and torso cut protection were not utilized where employees were exposed to shards of glass while disposing of waste raw glass in a dumpster.
