29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about January 27, 2017, and at times prior thereto, two power cords on the Hercules Elevator had the outer sheathing pulled back with exposed inner wires, exposing employees to the hazard of electrical shock.

29 CFR 1926.552(b)(3): Overhead protection covering of 2-inch planking, 4-Mar inch plywood, or other solid material of equivalent strength was not provided on the top of every material hoist cage or platform: On or about January 27, 2017, a cable guide bracket from the tower of the Hercules Elevator fell approximately 25 floors and almost struck an employee standing on the platform that was not completely covered by overhead protection, exposing employees to the hazard of being struck by falling material or parts of the elevator/hoist.
