29 CFR 1910.1200(e)(1): The employer did not implement a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met. Employees were not protected from hazards by appropriate work practices, emergency procedures and/or personal protective equipment. (Construction Reference: 1926.59) On or about February 21, 2018, and at times prior thereto, the employer did not evacuate employees in a timely manner to prevent exposure to high levels of carbon monoxide in the workplace. Measured levels of carbon monoxide exceeded 500 parts per million.
