29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): The employer allows workers to be exposed to hazardous conditions created by not conducting frequent or regular inspections. The most recent occurrence was at 5767 Hoffman Ford Court, St. Charles, MO, where they failed to make frequent or regular inspections of the worksite to ensure all workers were in compliance with company safety and health policies and OSHA standards. The workers were exposed to the following hazards at the job site: a) A worker was not wearing a protective helmet while exposed to struck-by hazards. b) A worker was exposed to fall hazards while performing siding installation from a ladder jack scaffold without a personal fall arrest system when the platform height was approximately 19 feet above ground.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system. Location: 5767 Hoffman Ford Ct, St. Charles, MO 63304 On 02/01/17, an employee from Modern Construction was exposed to an approximate 19 foot fall hazard as he was installing vinyl siding from a ladder jack scaffold without donning a personal fall arrest system. Modern Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(i), which was contained in OSHA inspection number 974819, citation number 1, item number 1, and was affirmed as a final order on 04/30/2014, with respect to a workplace located at 75 Shire Drive, Lake St. Louis, MO 63367.
