29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 22, 2017, the employer did not establish and utilize an adequate energy control procedure for employees to safely troubleshoot stoppages, clear fibers and unjam terminal strips on Terminal Machine 4-2 in the cutting area. Employees were exposed to amputation hazards while performing these tasks. No abatement certification or documentation is required for this item.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about January 22, 2017, the employer did not ensure that employees troubleshooting stoppages, clearing fibers and unjamming terminal strips on the Termination Machine 4-2 in the cutting area were protected from unexpected startup of moving and rotating components by performing the following sequence of actions prior to servicing the machine: 1) Machine or equipment shutdown 2) Machine or equipment isolation 3) Lockout or tagout device application 4) Verification of isolation Employees were exposed to amputation hazards while performing these tasks. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).
