29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Ram Packer, Line 1: On or about May 24, 2017, and at times prior, the employer exposed employees to caught-in/amputation  hazards, in that inward running nip points between wheels and track of the puller were not guarded to protect from  inadvertent contact with the wheels.

29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  a) Ram Packer, line 1: On or about May 24, 2017, and at times prior, the employer exposed employees to various physical and health hazards, in that a written personal protective equipment (PPE) assessment was not performed for employees working around vehicles and equipment to include, but is not limited to: forklifts; moving ram packing machinery; and nuisance dust from insulation.

29 CFR 1910.134(k): Training and information. This paragraph requires the employer to provide effective training to employees who are required to use respirators. The training must be comprehensive, understandable, and recur annually, and more often if necessary. This paragraph also requires the employer to provide the basic information on respirators in Appendix D of this section to employees who wear respirators when not required by this section or by the employer to do so.   a) Production floor: On or about May 24, 2017, and at times prior, the employer exposed employees to respiratory hazards, in that the information in Appendix D of the Respiratory Protection Standard was not provided to employees while using respirators for nuisance dust voluntarily.
