29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours.  On Sunday, 29 January 2017 at approximately 6:00 PM CST, an employee working within the Wareroom fell from the top of the second stack of palletized product to the concrete floor, sustaining injuries which required transport and hospitalization of the employee. The employee suffered a T11 wedge fracture and T5 endplate fracture to their spinal column, requiring surgery. Employee was admitted on 29 January and released on 31 January. First notification to OSHA was Tuesday, 31 January 2017 at 1:39 PM MDT.
