Stairs or a ramp were not provided where the exit route was not substantially level: Door #13: On or about May 2, 2018, and at times prior thereto, employees were exposed to a fall hazard in the event of an emergency, as the emergency exit included a fixed ladder on the exterior of the building for egress.

On or about May 2, 2018, and prior thereto, employees were exposed to a struck-by hazard while unjamming the Cram A-Lot compactor without Lock-Out/Tag-Out procedures developed specifically for it.
