29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition:    On or about February 1, 2017, and times prior to,    (a)  The concrete floor was broken and damaged along the diamond plating in Rod Storage leading into the Pickling area.      (b)  A hose was scattered on the floor at the northeast end of the Pickling area.     (c)  In the Course Draw area the handles on the floor covers for the electrical outlets that supplied power for the portable welders protruded approximately 2" from the floor.      (d)  The concrete floor was chipped and damaged along the steel plating over the trench in the Acid Cleanout Plating area.      (e)  The diamond plating was slightly jolted from the floor in the Fine Draw area where spools of wire were stored.      These conditions exposed employees to the hazard of slips, trips and falls from the same elevation.    Waste Water Facility:    (f)  There was a residual build-up of a sodium hydroxide solution adhered to the outside of the piping system to the waste caustic tanks.  This condition exposed employees to chemical skin irritations.

29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean, and to the extent feasible, a dry condition:    On or about February 1, 2017, and times prior to,     (a)  There was an excessive accumulation of water on the floor at the northeast end of the Pickling area.      (b)  There were openings in the roof in the Pickling area which allows water to accumulate on the floor when it rains.      (c)  In the Plating department the copper solution contained in the Anode bath caused the plastic grate that employees accessed to perform tasks such as but not limited to changing out copper plates in the Anode trays and checking and repairing cathode wires, to be slippery.      These conditions exposed employees to the hazard of slips, trips and falls.

Employees were exposed to falls from elevations.    (b)  On the two platforms in the Waste Water facility that employees accessed to monitor the waste caustic tanks.  Employees were exposed to falls from elevations.    In the Packing Department:    (c)  The ends of the elevated platform on the ASRS that employees used to reach upper carousels and work from to perform servicing tasks, were not guarded and no other measures were implemented to protect employees from falls.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points or rotating parts:    On or about February 1, 2017, and times prior to,    (a)  Employees were not prohibited from traveling in the vicinity of unguarded spools and wire in the PP Payoff area.  Employees were exposed to contact with rotating parts, drawing wire and nip points.    (b)  Employees were not prohibited from traveling beneath wires coming from PP Payoff going into the Furnace entrance area.  Employees were exposed to contact with drawing wire.    (c)  In the Course Draw area, employees reach a pipe beneath the gate guard on the course draw machines to hold down wire to connect properly to rotating blocks.  This practice exposed employees to inadvertent contact with rotating parts.    (d)  In the PP Storage Standing area, clam shell machines were not guarded to prevent employees for opening the standing hood while wire is twisting/stranding on the spool.  Employees were exposed to struck-by hazards.    (e)  In the Stranding area, employees were not prohibited from traveling in the vicinity of unguarded spools and wire on the stranding machines.  Employees were exposed to contact with rotating parts, drawing wire and nip points.
