On or about December 27, 2016, during the installation of roofing material at a job site located at 3900 Greenwich Road, Norton, Ohio, the employer did not ensure employees working on a roof of a single floor storage facility were protected from a fall of approximately 11 feet by utilizing a fall protection system.
