29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about October 3, 2019, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 7286 Virginia, Atwater, Ohio 44201.  Employees were working on lower levels below other employees that were using tools and materials and the employer did not ensure that they were wearing protective helmets.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about October 3, 2019, employees were exposed to a fall of approximately 13 feet while performing roofing activities on a residential home located at 7286 Virginia, Atwater, Ohio 44201.  The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.  Prime Exterior Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1207863, citation number 1 item number 1 and was affirmed as a final order on 2/23/17, with respect to a workplace located at 2313 Comet Road, Clinton, Ohio 44216.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about October 3, 2019, employees were exposed to a fall of approximately 10 feet while performing roofing activities on a residential home located at 7286 Virginia, Atwater, Ohio 44201.  The employees were using an extension ladder to access the roof and the employer did not ensure that it extended three feet above the landing area.  Prime Exterior Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1207863, citation number 1 item number 2 and was affirmed as a final order on 2/23/17, with respect to a workplace located at 2313 Comet Road, Clinton, Ohio 44216.
