29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points.  a) At the US Penitentiary, Recreation area supervisor's office, exit access was reduced to 20 inches, employee had to maneuver around the desk and furniture to get out through the narrowed exit access, employee was subjected to fire hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  a) At the US Penitentiary, Chemical Room area, the eye wash station was obstructed by plastic trash containers and not inspected weekly, the last inspection date was January 12, 2017.  b) At the US Penitentiary, CMS area, the eye wash station was not inspected weekly, the last inspection date was January 11, 2017.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment.  a) At the US Penitentiary, HVAC Shop, the energized wall outlet box used to plug-in electrical equipment was missing a knock-out.  b) At the General Maintenance area, the energized wall outlet box used for testing washers and dryers was missing a knock-out.  Employees were exposed to potential electric hazards or electric shock.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  a) At the US Penitentiary, Education area teacher's office, a black extension cord was used to connect a surge protector that is feeding computers and monitors for more than one year.  Improper use of flexible cord exposes employees to electrical  hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a) At the US Penitentiary, Receiving and Discharge area, two yellow flexible cords used to extend power to electrical equipment had damaged insulation, outer sheathing was taped, exposing employees to electrical hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  a) At the US Penitentiary Laundry Staff's Office, a re-locatable power tap was used to plug-in a refrigerator, a shredder and a microwave oven, overloading re-locatable power tap would subject employees to electrical fire hazards.  b) At the Education Technicians area, a re-locatable power tap was used to plug-in a microwave oven, a refrigerator and a coffee pot.  c) At the Recreation storage area, a re-locatable power tap was used to plug-in a microwave oven and an air compressor.  d) At the Housing Unit 1A, re-locatable power taps used for computers and monitors were daisy chained, one re-locatable power tap was connected to the other.  The Federal Bureau of Prisons was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 524558, citation number 1, item number 1 and was affirmed as a final order on 8/15/2012, with respect to a workplace located at 5701 8th Street, Dublin, CA 94568.
