29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points.  a) At the Federal Correctional Institute -2, Education Office' rooms EM 140 and 144, the exit access was reduced to 16 and 27 inches, employees had to maneuver around the desk and furniture to get out through the narrowed exit access, employee was subjected to fire hazards.  b) At FCI-2 Commissary area sale station, the exit access was reduced to 14 inches, employees had to maneuver around the counter and the freezer to get out through the narrowed exit access.  c) At FCI-2 Special Housing Unit, E-138 counselor's office, exit access was reduced to 23 inches, employees had to maneuver around the desk and furniture to get out through the narrowed exit access.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.   a) At FCI -2 Tool room, the portable fire extinguisher was obstructed by trash container, it was not available for immediate use by employees.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a) At the Federal Correctional Institute -2, Facilities General Maintenance Shop area, the Wilton drill press did not have a guard exposing employees to rotating parts.

29 CFR 1910.212(a)(5): Fan blade guards were not provided where the periphery of the blades was less than seven feet above the floor or working level and had openings larger than one half inch:  a) At the Federal Correctional Institute -2, Facilities general area, the industrial fan used for air circulation energized in the floor area had an opening of about five (5) inches exposing employees to the rotating fan blades.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment.  a) At the Federal Correctional Institute 2, Balers B2-02-VIX and S-01 used for compaction were missing knock-outs in their control panels, exposing employees to potential electric hazards or electric shock.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a) At FCI -2, Facilities general maintenance shop area, the power snake's cord used for un-clogging utilities had damaged insulation, outer sheathing was torn and it is missing its ground prong, exposing employees to electrical shock hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  a) At the Federal Corrections Institute -2, EF-149 unit conference room, a re-locatable power tap was used to plug-in a refrigerator, a coffee pot and a microwave oven, overloading re-locatable power tap would subject employees to electrical fire hazards.  b) At FCI-2 Commissary sale station, a re-locatable power tap was used to plug-in a refrigerator.  c) At FCI-2 Education Tech's Office,  re-locatable power taps used for computers and monitors were daisy chained, one re-locatable power tap was connected to the other.  d) At FCI-2 Laundry services area, re-locatable power taps used sewing machines were daisy chained, one re-locatable power tap was connected to the other.  e) At FCI-2 Receiving and Discharge records office, re-locatable power taps used for inmate management systems were daisy chained, one re-locatable power tap was connected to the other.  The Federal Bureau of Prisons was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 524558, citation number 1, item number 1 and was affirmed as a final order on 8/15/2012, with respect to a workplace located at 5701 8th Street, Dublin, CA 94568.
