29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points.  a) At the Federal Prison Camp Dormitory Counselor's Office, exit access was reduced to 22 inches, employees had to maneuver around the desk and file cabinet to get out through the narrowed exit access, employees were subjected to fire hazards.  b) At the Recreation Office, exit access was reduced to 23 inches, employees had to maneuver around the desk and furniture to get out through the narrowed exit access.  c) At Staff Psychologist Office, exit access was reduced to 25 inches, employees had to maneuver around the desk and furniture to get out through the narrowed exit access.  d)) At Commissary Warehouse Supervisor's Office, exit access was reduced to 26 inches, employees had to maneuver around the desk and furniture to get out through the narrowed exit access.

29 CFR 1910.165(e): Manually operated actuation devices for use in conjunction with employee alarms were not unobstructed, conspicuous and readily accessible.  a) At the Federal Prison Camp Recreation area indoor cardio, the two manual pull fire actuation alarms were obstructed by the inflated yellow balls, they were not readily accessible by employees.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective.   a) At the Federal Prison Camp Dormitory Photography room, the power cord cart used for electronic equipment did not have a ground pin, exposing employees to electrical hazards.  b) At the Staff Psychologist's Office, the power cart used to plug-in TV and VCR was missing a ground pin.   c)  At the Food Service Scullery area, the fly diffuser light was missing a ground pin.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws.  a) At the Federal Prison Camp Receiving and Discharge, the Wilson Jones shredder did not have strain relief, flexible cord's insulation was being pulled away from its outer sheathing.  Employees were exposed to electrical shock hazards.  b) At the Camp Garage, the brake lathe machine did not have strain relief, flexible cord's insulation was being pulled away from its outer sheathing.  Employees were exposed to electrical shock hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling.  a) At the Federal Prison Camp Dormitory Counselor's Office, re-locatable power taps used for computers and a refrigerator were daisy chained, one re-locatable power tap was connected to the other.  Overloading re-locatable power tap would subject employees to electrical fire hazards.  b) At the Chaplain's Office, a re-locatable power tap was used to plug-in computers and a refrigerator.  c) At the Drug and alcohol coordinators office,  re-locatable power taps used for computers and monitors were daisy chained, one re-locatable power tap was connected to the other.  d) At the Education Culinary Arts Office, a re-locatable power tap was used for coffee pot and microwave oven.  e) At the USP Power House, a re-locatable power tap was used for a toaster oven, a refrigerator and a waffle maker.  The Federal Bureau of Prisons was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 524558, citation number 1, item number 1 and was affirmed as a final order on 8/15/2012, with respect to a workplace located at 5701 8th Street, Dublin, CA 94568.
