29 CFR 1926.652(e)(2)(i): Excavation of material to a level no greater than 2 feet (.61m) below the bottom of the members of the support system was allowed when there were indications while the trench was open of a possible loss of soil from behind or below the bottom of the support system:      On or about February 7, 2017, at an excavation site, employees were observed working in a 23 foot deep trench with the bottom of protective system elevated more than two feet above the trench floor.  The employees were exposed to struck by and crushed by hazards.

29 CFR 1926.652(g)(1)(iii): Employees were not protected from the hazard of cave-ins when entering or exiting the area protected by shields:                                                                                                                         On or about February 7, 2017, at the trenching site located at 3800 S.E. 29th in Oklahoma City, OK, employees were not protected from cave-ins while using a ladder to enter and exit the excavation. The employees were exposed to struck-by and crushed-by hazards.
