29 CFR 1910.28(b)(11): (11) Stairways.  The employer must ensure:    29 CFR 1910.28(b)(11)(ii): (ii) Each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as follows: (see TABLE D-2)    a. Located on the upper stairway leading from the Wayne Foam Line to the second floor storage mezzanine and offices: The employer did not ensure that the stairway had an intermediate rail installed along the open sides of the stairway to protect employees from falling approximately twelve feet to the ground below.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.   a. Located at Shaper 8 in the Woodworking Department: On or about January 31, 2017, and at times prior, employees were operating a Grizzly, model G1026, shaper machine to cope edges of thermoplastic garage door trim material. The cutting tool of the machine was not guarded to protect the operator from amputation hazards.      b. Located at Shaper XX in the Woodworking Department: On or about January 31, 2017, and at times prior, employees were operating a Grizzly, model G1026, shaper machine to cope edges of thermoplastic garage door trim material. The cutting tool of the machine was not guarded to protect the operator from amputation hazards.      c. Located at Shaper 14 in the Woodworking Department: On or about January 31, 2017, and at times prior, employees were operating a Shop Fox, model W1674, shaper machine to shape edges of thermoplastic garage door trim material. The cutting tool of the machine was not guarded to protect the operator from amputation hazards.      d. Located at Shaper 13 in the Designer Department: On or about January 31, 2017, and at times prior, employees were operating a Shop Fox, model W1674, shaper machine to cope edges of thermoplastic garage door trim material. The cutting tool of the machine was not guarded to protect the operator from amputation hazards.      e. Located at Shaper 15 in the Designer Department: On or about January 31, 2017, and at times prior, employees were operating a Shop Fox, model W1674, shaper machine to cope edges of thermoplastic garage door trim material. The cutting tool of the machine was not guarded to protect the operator from amputation hazards.      f. Located at Shaper - in the Atlanta Door Department: On or about January 31, 2017, and at times prior, employees were operating a Grizzly, model G1026, shaper machine to cut a strip along the edge of garage door trim materials. The cutting tool of the machine was not guarded to protect the operator from amputation hazards.
