29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On February 1, 2017 the guarding was not attached to the Digicon 3000 label machine, exposing employees to pinch points hazards from the rotating rollers.

29 CFR 1910.219(c)(3): Guarding vertical and inclined shafting. Vertical and inclined shafting seven (7) feet or less from floor or working platform, excepting maintenance runways, shall be enclosed with a stationary casing in accordance with requirements of paragraphs (m) and (o) of this section. a) The mixing shaft attached to the motor drive that was going into a bucket for the gluing material was not enclosed to prevent employees exposure to rotating parts.
