29 CFR 1926.1053(b)(22): Employees  using ladders were carrying objects or loads that could cause the employee to lose balance and fall:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)     a) Exterior Wall - Oak Ave. Façade - Above Garage: Employee was exposed to a fall hazard of approximately 14 feet while carrying a large piece of plywood as he climbed an extension ladder, on or about 1/7/17.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Exterior of Building - Flat Roof By Garage Doors: Employees without any means of fall protection were exposed to fall hazards of approximately 14 feet while engaged in framing activities, on or about 1/7/17    VMV Carpentry Inc. was previously cited REPEAT for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 986173, citation number 02, item number 01 and was affirmed as a final order on 9/30/14, with respect to a workplace located at 150 Rahway Rd, Scotch Plains NJ.  VMV Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 461433, citation number 01, item number 02 and was affirmed as a final order on 2/1/13, with respect to a workplace located at 160 Fern Ave., Lyndhurst NJ.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) Exterior of Building - Flat Roof By Garage Doors: Employee was exposed to a fall hazard of approximately 14 feet while climbing an extension ladder that only extended approximately one foot over the working level, on or about 1/7/17.    VMV Carpentry Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 986173, citation number 01, item number 02 and was affirmed as a final order on 9/30/14, with respect to a workplace located at 150 Rahway Rd, Scotch Plains NJ.
