29 CFR 1926.501(b)(14): The employer did not ensure that each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, were not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.  a) 301 Nott Street, Schenectady - on February 8, 2017, and at times prior, guardrails installed for fall protection at wall openings had no midrails, exposing employees to falls of 15 to 80 feet.  b) 301 Nott Street, Schenectady - on February 8, 2017, and at times prior, cables installed around unprotected edges were too low, exposing employees to falls of 15 to 80 feet.
