No opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension: On or about February 10, 2017, in the UNICOR facility press area, a stairway with 13 risers leading up to the press foreman's office was not provided with a stair rail in that the railing did not have a midrail leaving large openings in the stair rail, there by exposing employees to a fall hazard.

29 CFR 1910.212(a)(1) as required by 1960.8(b): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about February 9, 2017, in the UNICOR facility tool and die area, Fortune 1640 Metal Lathe, was not equipped with a guard at the rotating chuck, exposing employees to a struck by, caught in and/or caught on hazard. b. On or about February 9, 2017, in the UNICOR facility tool and die area, Fortune 1640 Metal Lathe, was not equipped with a guard at the hole through spindle, exposing employees to a struck by, caught in and/or caught on hazard.

29 CFR 1910.242(b) as required by 1960.8(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: On or about February 10, 2017, in the UNICOR facility tool and die area, the employer did not reduced the air pressure to 30 p.s.i. on air nozzles used for cleaning parts. The air pressure was set at approximately 100 p.s.i., there by exposing employees to a struck by hazard.
