29 CFR 1910.137(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and the maximum interval between tests shall be in accordance with Table I-5 and Table I-6:     a) On or about February 8, 2017, at the above located workplace, employees required to work with 480 volt electricity used electrical insulating gloves.  The gloves were not subjected to periodic electrical tests in accordance with Table I-5 and Table I-6. Employees were thereby exposed to hazards associated with live electrical parts.

The employer's procedure did not establish and require the sanitation employees to lockout the equipment before performing service and maintenance as required by 29 CFR 1910.147(d)(1)-(6).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:          a) On or about February 7, 2017, at the above located workplace, machine operators and sanitation workers were required to add ingredients to and clean dough mixer number 2 (a Peerless roller bar mixer). The mixer's cover had an ingredients door opening which was unguarded. Employees were thereby exposed an amputation and/or crushing hazard.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there was potential electrical hazards were not provided with, and use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed.       a) On or about February 8, 2017, at the above located work place, employees were required to perform voltage testing and working on 480 volt electrical equipment. The employer did not provide and require employees to use the following appropriate Arc-Flash Rated Protective Clothing (Flame resistant (FR) clothing). Employees were thereby exposed to hazards associated with electricity and electrical explosions.

29 CFR 1910.333(b)(2)(i): The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) of 29 CFR 1910.333 and did not make it available for inspection by employees and the Assistant Secretary of Labor and his or her authorized representatives:  a) On or about February 7, 2017, at the above located workplace, employees were required to operate and perform maintenance on equipment such conveyor belts. The employer did not maintain a copy of the written procedures outlined in 29 CFR 1910.333(b)(2) and did not make it available for inspection by employees. The employees working with the equipment were thereby exposed to the hazards associated with the unexpected energizing or startup of equipment.
