29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) On or about August 23, 2017, at the above located workplace, machine operators were required to service and/or maintain equipment such as the AMF Bakery Systems, High-speed Bun Divider/Pan-O-Mat, Conveyors/Pan Indexer and the associated equipment. The employer had not trained the employees in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. The East Balt Bakeries was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(7)(i)(A), which was contained in OSHA inspection number 1209232, citation number 1, item number 3 and was affirmed as a final order on April 19, 2017, with respect to a workplace located at 1801 W. 31st Street, Chicago, IL 60608.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about August 23, 2017, at the above located workplace, machine operators were required to service and/or maintain equipment such as the AMF Bakery Systems, High-speed Bun Divider/Pan-O-Mat, Conveyors/Pan Indexer and the associated equipment. The employer's unjamming procedure did not establish and require the machine operators to lockout the equipment before cleaning and/or unjamming as required by 29 CFR 1910.147(d)(1)-(6). The East Balt Bakeries was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d), which was contained in OSHA inspection number 1209232, citation number 1, item number 4 and was affirmed as a final order on April 19, 2017, with respect to a workplace located at 1801 W. 31st Street, Chicago, Il 60608.
