29 CFR 1926.451(e)(9)(i): The employer did not provide safe means of access for each employee erecting or dismantling a scaffold: a) On the Stanwix Street side of the job site, on or about February 10, 2017 - The employer failed to provide a safe means of access to the work platform from the base of a Fraco hydraulic mast-climbing scaffold while employees were erecting the scaffold. Employees were exposed to fall, struck, and caught between hazards while climbing the mast to reach the work platform.
