1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a.) Vertical Access Solutions LLC; 1 Llodio Dr, Hermitage, PA 16148: On or about 02/16/2023 and at times prior thereto; employees were installing decking on the West side scaffolding at heights of approximately 16-feet without fall protection. Employees were observed wearing harnesses without attaching to an anchor point.
