29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a). Worksite located at 517 E. Highway 24, Independence, Missouri- Employee was observed not wearing the fall harness correctly and did not have the lanyard connected to the boom or basket of the lift, therefore, he was not protected from falling. The KC Sign Express, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(iv), which was contained in OSHA inspection numbers 1008142, citation number 1, item numbered 1, and was affirmed as a final order on December 12, 2014, with respect to a workplace located at 8383 Booth Road, Kansas City, Missouri 64157. The KC Sign Express, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(iv), which was contained in OSHA inspection numbers 950501, citation number 1, item numbered 2, and was affirmed as a final order on March 13, 2013, with respect to a workplace located at 5520 N.
