On or about 2/7/17 - PE-X laboratory; A quality control test which utilized xylene, a flammable liquid, was conducted in a laboratory hood without an operating exhaust fan. b. On or about 2/7/17 - PE-X laboratory; The laboratory hood was not equipped with a flow indicator to alert users to improper exhaust flow.

On or about 2/07/17 - Automotive/PE-X Lab, employee training did not address specific work practice controls such as the assurance of fume hood operation throughout testing, the physical hazards associated with the use of xylene, a flammable liquid, and the actions to take in the event of a malfunction of the fume hood and/or a chemical spill.
