Violation observed on or about February 14, 2017.  b) At 958 Terra Vista Street, employees were exposed to a 11 foot fall hazard, in that, guardrail systems, safety net system, or personal fall arrest system were not used. 29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) At 954 Terra Vista Street, 5 employees were exposed to a 11 foot fall hazard, in that, an extension ladder was used by the employees to access and descend from the second floor, extended approximately 12 inches above the landing surface. This violation was observed on or about February 14, 2017.  b) At 958 Terra Vista Street, 5 employees were exposed to a 11 foot fall hazard, in that, an extension ladder was used by the employees to access and descend from the second floor, extended approximately 12 inches above the landing surface. This violation was observed on or about February 14, 2017.

This violation was discovered on or about February 14, 2017.
