29 CFR 1910.28(b)(1)(i) as required by 1960.8(b): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:   On or about February 14, 2017, in the East Shelter, employees were working within one foot of unguarded edge while removing safety stanchions and chains from the edge of a stop log and the employer did not ensure employees were protected from a fall of 8 ½ feet to the ground.

29 CFR 1910.252(a)(2)(iii)(A) as required by 1960.8(b): The employer did not provide fire watchers whenever welding or cutting was performed in locations where other than a minor fire might develop:    On or about February 14, 2017, the employer did not post a fire watch in the west shelter while employees performed hot work operations on Stop log, when slag, sparks, weld splatter can come into contact with combustible material such as but not limited to plywood and could cause a fire.

29 CFR 1910.157(g)(3) as required by 1960.8(b): The employer shall provide employees who have been designated to use fire fighting equipment as part of an emergency action plan with training in the use of the appropriate equipment.   On or about February 14, 2017, the employer did not provide annual training on portable fire extinguishers that included hands-on training to employees designated to use portable fire extinguishers.    U.S. Army corps of Engineers, Soo Area Office was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.157(g)(3), which was contained in OSHA inspection number 1047681, citation number 1, item number 9 and was affirmed as a final order on November 23, 2015, with respect to a workplace located at 315 W. Portage Ave, Sault Ste. Marie, Michigan 49783.

29 CFR 1910.303(b)(2) as required by 1960.8(b): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  On or about February 14, 2017, the employer did not ensure that extension cords were not daisy chained to other extension cords. On the outside of the east shelter, three extension cords were daisy chained together two provide power to exhaust fan for the east shelter.
