29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Packaging Department, Line 21 - On or about 2/14/2017, and at times prior, employees were exposed to amputation hazards while employees were packing the tubes off the Prosys machine conveyor. Broken flat conveyor segments created openings that resulted in shear points when these openings and adjoining segments passed under the end guard of the conveyor.
