29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about February 14, 2017, employees were exposed to being struck by flying objects. The employees were performing framing activities on a new residential home in Medina, Ohio 44256. An employee was using a reciprocating saw to cut framing materials without wearing safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE. On or about February 14, 2017, employees were exposed to a fall of approximately 11 feet. The employees were performing framing activities on a new residential home in Medina, Ohio 44256. The employees were on the roof without using any form of fall protection.
