29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about October 1, 2018, during the installation of framing material at the job site located at 6171 Llanfair St SW, Massillon, Ohio 44646; the employer did not ensure employees working on the roof of a new construction residential single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing framing/roofing material.     The Grand Valley Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.102(a)(1) Eye and Face protection), which was contained in OSHA inspection number 1210748, citation number 01, item number 01 and was affirmed as a final order on 3/23/2017, with respect to a workplace located at 5552 Firth, Medina, Ohio 44256.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a)  NOTE TO CSHO: USE THIS OPTION ONLY IN CASES WHERE THE INFEASIBILITY/GREATER HAZARD EXCEPTION IS NOT IN ISSUE.   On or about October 1, 2018, during the installation of roofing material at a job site located at 6171 Llanfair St SW, Massillon, Ohio 44646; the employer did not ensure employees working on a roof of a single family home were protected from a fall of approximately 15.4 feet by utilizing a fall protection system.     The Grand Valley Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1210748, citation number 01, item number 02 and was affirmed as a final order on 3/23/2018, with respect to a workplace located at 5552 Firth, Medina, Ohio 44256.
