29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: On or about February 16, 2017, employees were exposed to burns and fractures while performing set up and servicing activities on the Line 1 Wave Solder Machine and the Line2 MV2 Panasonic Machine when the employer did not ensure that an energy control program was established which included documented specific procedures, employee training, and periodic inspections. Abatement certification is required in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about February 16, 2017, the employer did not ensure that employees performing set up and servicing activities on the Line 1 Wave Solder Machine and the Line 2 MV2 Panasonic Machine were protected from the unexpected startup of moving and rotating components by performing the following sequence of actions prior to servicing the machine: 1) Machine or equipment shutdown 2) Machine or equipment isolation 3) Lockout or tagout device application 4) Verification of isolation. Employees were exposed to burns and fractures when performing these tasks. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).

Employees were operating powered industrial vehicles to move supplies and scrap to recycling bins and vehicles.
