Joints were not riveted, welded or made tight by some equally effective means. The door was not provided with a three-point lock, and the door sill was not raised at least 2 inches above the bottom of the cabinet:    a) In the Machining Area, on or about February 15, 2017 - The metal cabinet used to store flammable liquids, including paint, was constructed of aluminum, was not double walled with at least 1 1/2 - inch air space, didn't have joints that were riveted, welded or made tight, and didn't provide a door with a three-point lock. The cabinet was made by the employer and contained a heater on the inside of the cabinet to prevent paint from freezing.
