Building 4 - On or about February 15, 2017, employees operated pneumatic nail guns without the use of eye protection, thus exposing employees to struck-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  a). Building 4 - On or about February 15, 2017, employees were installing 2nd floor trusses while working from already installed floor trusses without a fall protection system in place, thus exposing employees to a fall hazard.  b) Building 6 - On or about February 15, 2017, employees were installing wood bracing around the edges of the 2nd floor without a fall protection system in place, thus exposing employees to a fall hazard.

Building 4 - On or about February 15, 2017, subcontractor employees used the top cap and top step of a Louisville (model: W-3215-06) and a Werner (model: FS206) step ladder to access/egress the 2nd floor trusses and walls, thus exposing employees to a fall hazard.
