29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) At the job site, on or about December 22, 2016, several employees who were performing framing work on a new single-story multi-family residence were exposed to fall hazards up to approximately 9 feet onto the 1st floor below because the workers were not utilizing any form of fall protection.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a) Outside stairs in front of the trailer office, on or about December 22, 2016 and at times prior to, the General Contractor did not ensure stairrails were installed prior to use, exposing employees and sub-contractors' employees to a fall hazard.
