29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure(s) at least annually to ensure that the procedure and the requirements of this standard are being followed:        The employer does not ensure a periodic evaluation of the lockout/tagout procedures with all authorized employees is performed annually for all equipment which the employee is authorized to lockout/tagout, such as, but not limited to, EMM press #43, AWT Press, coil shear, exposing employees to caught-in and struck by hazards.      Polyvision Corporation was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA  inspection number 1078749, Citation number 1, Item number 3b, and was affirmed as a final order on February 8, 2016 with a final abatement date February 8, 2016, with respect to a workplace hazard at 4301 N. Wood Drive, Okmulgee, Oklahoma 74447.        Pursuant to 29 C.F.R 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that authorized employees receive a periodic inspection of the machines/equipment they are authorized to lockout/tagout.

29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      The employer does not ensure adequate machine guarding is in place to protect employees along Line #4 and Line #5.  Employees are exposed to hazards such as, but not limited to moving product, hot contact surfaces, ingoing nip points.      Polyvision Corporation was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA  inspection number 1078749, Citation number 1, Item number 5, and was affirmed as a final order on February 8, 2016 with a final abatement date of January 30, 2017, with respect to a workplace hazard at 4301 N. Wood Drive, Okmulgee, Oklahoma 74447.      Pursuant to 29 C.F.R 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that machine guarding is in place and remains in place.

29 CFR 1910.307(c):  Electrical equipment in hazardous (classified) locations is not intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) locations:                    The employer does not ensure that equipment is intrinsically safe and approved for the hazardous location when employees are operating the screen print wash station, the back lighting was not approved for the Combustible Class II Liquid being used to wash the screens exposing employees to the hazards of flash fires.         Polyvision Corporation was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.307(c), which was contained in OSHA inspection number 1078749, Citation number 1, Item number 7b, and was affirmed as a final order on February 8, 2016 with a final abatement date of January 20, 2016, with respect to a workplace hazard at 4301 N. Wood Drive, Okmulgee, Oklahoma 74447.      Pursuant to 29 C.F.R 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that electrical equipment in hazardous locations is approved for that hazardous classification.

Polyvision does not ensure that the OSHA 300A Annual Summary of Injuries and Illnesses for 2016 includes all recordable injuries and recordable standard threshold shifts during 2016.    Pursuant to 29 C.F.R 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that OSHA recordable injuries and recordable standard threshold shifts are included on the OSHA 300A Annual Summary of  Injuries and Illnesses.

The employer does not ensure that employees who are certified to operate powered industrial trucks are evaluated every three years to ensure the operators continued safe operation of the forklift exposing employees to struck-by hazards.       Pursuant to 29 C.F.R 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that certified powered industrial truck operators are evaluated every three years to ensure safe operation of the forklifts.
