29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)  Jack Shop:  On or about February 21, 2017 lathe operators were permitted to lift the chuck guard and then secure and/or remove work pieces into and out of the chuck on the MAZAK lathes without utilizing the procedures for de-energizing the equipment.  This exposed the lathe operators to being struck by heavy metal work pieces and being pulled into rotating parts if the lathe's spindle was accidentally started.
