29 CFR 1910.244(a)(2)(iii): Loads were not cribbed, blocked or otherwise secured immediately after being raised with a jack. a) Jobsite: Container MRKU5802384 - On February 21, 2017, an employee was working under a Toyota Sienna Le minivan that was not was not cribbed, blocked, or otherwise secured after it was raised with a Pittsburgh Automotive floor jack. While working under the elevated vehicle, the automobile fell onto the employee causing fatal injuries.

29 CFR 1910.244(a)(2)(viii): Jacks which were out of order were not tagged accordingly, and were used without repairs being made. a) Jobsite: Container MRKU5802384 - A Pittsburgh Automotive Floor Jack that was missing its rear casters and load rating label was used during car loading operations. The use of the damaged jack exposed employees to falling object and crushing hazards. Observed on or about 2/21/17.

The floor jack was missing the rear casters. Observed on or about 2/21/17.
